YMCA of Greater Seattle Whole Person Health Navigation Coordinator in Seattle, Washington

Directly supports the Whole Person Health Navigation Hub by conducting data entry, tracking, and reporting needed to connect referred individuals to the YMCA’s suite of wellness programs, such as chronic disease prevention and management programs, wellness coaching, behavioral health counseling, and others. Conducts phone- and email-based outreach to these individuals, schedules and/or enrolls them in appropriate programming, and maintains accurate records for data collection and reporting purposes. Provides administrative, organizational, and logistical support for all healthy living services, and to the Navigation Hub and the Health and Social Services Center of Excellence as a whole.

YGS will comply with all WA State mandates regarding COVID vaccinations.

All YGS staff in Child Care roles will be required to be fully vaccinated for COVID by October 18, 2021. All YGS staff will be required to be fully vaccinated for COVID by November 1, 2021 regardless of work location. Religious and Medical Exemptions will be considered at time of hire.


  1. Maintains an electronic database of private health information and enters and sends/receives data through the referral management system following all privacy and security protocols.

  2. Conducts care coordination conversations with referred individuals and other prospective community members utilizing motivational interviewing and active listening skills.

  3. Works in collaboration with other Centers of Excellence to coordinate referral management, new membership, and program enrollments.

  4. Ensures accurate intake forms, data collection forms, and appropriate program documentation are completed.

  5. Supports quality assurance and other administrative duties related to Health and Social Services programming across the Association.

  6. Other duties as assigned.

This job operates in a professional office environment. This role routinely uses standard office equipment.


The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position requires the ability to occasionally lift office products and supplies, up to 20 pounds.


This is a full-time position, and hours of work and days are Monday through Friday, 8:30 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job duties demand.


Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.



  1. One or more years of education and/or work experience in health promotion, public health, community outreach, or similar field.

  2. Working knowledge of preventative health programs and philosophy.

  3. Comfort interacting with individuals’ health-related information and communicating with health care providers.

  4. Intermediate to advanced computer skills and experience with Microsoft Office suite.


  • One or more years of college education in health promotion, public health, or a related field. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.

  • Prefer knowledge of and previous experience with diverse populations (language, culture, race, physical ability, sexual orientation, etc.).

  • Prefer any type of previous experience interacting with the healthcare system.

  • Proven history of successfully delivering projects on time and within budget.

  • Demonstrated excellent oral and written communication skills.

  • Ability to work with creativity and flexibility, both independently and as a team member.

  • Ability to speak any language in addition to English.
